Elderly individuals and their families can apply for services and arrange for unified assessment through nearby Family Service Centers / Integrated Family Service Centers, Medical Social Services Departments, Elderly Services Centers, Elderly Neighborhood Centers, or Elderly Activity Centers. Upon assessment by under the Standardised Care Need Assessment Mechanism for Elderly Services and categorized as moderately or severely physically impaired seniors, they will be assigned to service teams for follow-up based on their residential areas.
