'Enhanced Home and Community Care Services provides services of integrated model for the frail elders to put into “Care for elderly” and “Continuous Care” practice, which can enable them to receive professional nursing services and personal care without leaving their home and community and maintain maximum activity level.
Elderly persons aged 65 or above living in the community and who are assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
Elderly individuals aged 60-64 may apply if assessed as needing services.
