Integrated Discharge Support Programme for Elderly Patients (HK East Cluster) provides a range of support services for high-risk elderly persons who are newly discharged from public hospitals, with an aim to reduce the rate of their unplanned readmission to hospital. We also provide support and training to carer to alleviate the stress in caregiving.
Our team of professionals, which includes social workers, nurses, physical therapists, occupational therapists, health workers, rehabilitation therapy assistants, personal care attendants, and domestic helpers, offers a range of home services to seniors who have been discharged from the hospital and their care providers. These services may include meal delivery, personal care, nursing care, counseling, rehabilitation therapy, and home modifications, among others.
